- Patients with active periodontal disease must have deep periodontal disease cleanings before being eligible for maintenance cleanings.
- This plan is not insurance and cannot be used with another dental plan or with dental insurance; for treatment which, in the sole opinion of our doctors, lies outside the realm of their capability; for referrals which are covered under automobile medical; or for services of injuries covered under worker’s compensation.
